After thoroughly reading the Dementia Friends USA Welcome Guide, please respond to the implementation questions below. Please speak to the following:

How will you identify your Master Champion who will self-training using the materials provided by Dementia Friends USA and develop an approach to train additional Champions statewide? What is your plan to identify Champions (volunteers, paid staff, etc) who are equipped to offer in-person Dementia Friends sessions?(Required)

Please provide an overview of your proposed implementation timeline (estimate when first round of Dementia Champions would be trained and when first Dementia Friends sessions would be held). Please share where/when do you plan to offer Dementia Friends sessions in your state. Please also outline your plans for tracking your monthly training numbers to DFA. Sublicense administrators report their monthly numbers to DFA via online survey.(Required)
